「通院が難しくなってきたのですが、どうしたらよいでしょうか」
この問いの背景には、現代社会の構造的な変化が深く関わっています。
今回は、「なぜ在宅医療なのか？」というテーマについて、現場の実感も交えながら整理してみたいと思います。

1．在宅医療に入る理由とは何か
在宅医療は、「特別な人のための医療」ではありません。
むしろ、現代の生活環境の変化に応じて、自然に求められるようになった医療の形だと感じています。
現場で多く見られる背景として、いくつかの要因があります。

① 核家族化による支援体制の変化
かつては、家族の中で高齢者を支える仕組みがありました。
しかし現在は、核家族化が進み、
・子どもが遠方に住んでいる
・日中に見守る人がいない
といった状況が増えています。
その結果、「通院を支える人がいない」という課題が生じています。

② 免許証返納による移動手段の喪失
高齢ドライバーによる事故が社会問題となる中、免許証を返納される方も増えています。
これは安全の観点では重要な判断ですが、その一方で、
「病院に行く手段がなくなる」
という新たな問題を生みます。
特に地方では、公共交通機関だけでの通院が難しいケースも少なくありません。

③ 生活基盤と交通環境のミスマッチ
富士市・富士宮市のように、自然豊かな地域では、
・坂道が多い
・バスの本数が限られている
といった環境もあり、身体機能の低下とともに通院が困難になるケースが見られます。
「住み慣れた場所に住み続けたい」という思いと、「医療へのアクセス」の間にギャップが生まれているとも言えるでしょう。

④ お薬の自己管理の難しさ
もう一つ、見落とされがちですが重要なのが「服薬管理」です。
・飲み忘れ
・重複服用
・残薬の増加
こうした問題は、体調の悪化や入院のリスクにもつながります。
「薬をもらえている＝治療が成立している」ではないという現実が、在宅医療の必要性を高めています。

2．在宅医療の良さとは何か
では、在宅医療にはどのような価値があるのでしょうか。
それは単なる「訪問サービス」ではなく、
生活の場そのものを医療の現場として捉える視点にあります。

在宅医療では、

・看護師
・薬剤師
といった専門職が、自宅に訪問します。
これにより、
「通院できないから医療が受けられない」という状況を回避することができます。

・食事内容

といった情報を、医療者が直接把握できるのが在宅医療の強みです。

高齢者が高齢者を支える「老老介護」の現場では、ご家族だけで抱え込むことに限界があります。
在宅医療を導入することで、

が可能となり、「支え合いの輪」が広がっていきます。

3．在宅医療のサービスを受けるには
では、実際に在宅医療を受けるにはどうすればよいのでしょうか。
基本的な流れは、次のようになります。

まず中心となるのが、ケアマネージャーの存在です。
患者様の状態や生活状況に応じて、
・どのサービスを
・どの頻度で
・どのように組み合わせるか
といった計画（ケアプラン）を作成します。
この計画に基づき、医療・介護サービスが提供されます。

② 医師の訪問診療と指示
在宅医療では、医師が定期的に訪問診療を行います。
そして、
その医師の指示のもとで、看護師や薬剤師が訪問する
という形で、チーム医療が構築されます。
薬剤師としては、
・処方内容の確認
・服薬指導
・副作用のモニタリング
・お薬カレンダーの調整
などを通じて、治療の質を支えています。

4．在宅医療は「特別」ではなく「これからの標準」
在宅医療というと、まだどこか特別なもののように感じられるかもしれません。
しかし現場の実感としては、
「必要な人にとって、最も自然な医療の形」へと変化しつつあります。

お薬の管理に不安が出てきたとき、
ご家族だけでの支えに限界を感じたとき。
そのタイミングで、
「在宅医療」という選択肢を知っているかどうか
は、その後の生活の質に大きく関わってきます。

私自身、在宅医療の現場で感じるのは、
医療とは単に病気を治すものではなく、「その人の生活を支える営み」であるということです。
そしてその中心は、常に患者様ご本人の生活にあります。
在宅医療は、その生活に寄り添うための、一つのかたちです。

Home medical care is not “medical care for special cases.”
Rather, I feel it is a form of care that has naturally emerged in response to changes in modern living environments.
There are several key factors commonly observed in practice.

① Changes in Support Systems Due to the Shift to Nuclear Families
In the past, there were built-in family structures that supported elderly members.
However, with the rise of nuclear families:
•Children live far away
•No one is available during the day to provide support
These situations are becoming more common.
As a result, the issue arises that
“there is no one to support hospital visits.”

② Loss of Transportation Due to Driver’s License Surrender
As traffic accidents involving elderly drivers become a social concern, more people are voluntarily returning their driver’s licenses.
While this is an important step for safety, it also creates a new challenge:
“losing the means to visit medical institutions.”
Especially in regional areas, relying solely on public transportation can make hospital visits difficult.

③ Mismatch Between Living Environment and Transportation Infrastructure
In areas like Fuji City and Fujinomiya City, where nature is abundant:
•There are many slopes
•Bus services are limited
As physical function declines, these factors can make hospital visits increasingly difficult.
It can be said that
a gap is emerging between the desire to continue living in a familiar place and access to medical care.

Another important but often overlooked factor is medication management.
•Missed doses
•Duplicate intake
•Accumulation of unused medications
These issues can lead to worsening health conditions and increased risk of hospitalization.
“Receiving medication does not necessarily mean that treatment is properly being carried out.”
This reality further highlights the necessity of home medical care.

What value does home medical care provide?
It is not merely a “visiting service,” but rather

In situations where elderly individuals care for one another, there are natural limits to what families can manage alone.

So, how can one actually begin receiving home medical care?

Based on the patient’s condition and living situation, they create a care plan that determines:

•How frequently they should be provided

Medical and caregiving services are then delivered based on this plan.

In home medical care, physicians conduct regular home visits.
Then,
based on the physician’s instructions, nurses and pharmacists provide their respective services through home visits.
As a pharmacist, my role includes:
•Reviewing prescriptions
•Providing medication guidance
•Monitoring for side effects
•Adjusting medication calendars
Through these activities, we support the overall quality of treatment.

4. Home Medical Care: Not Exceptional, but a New Standard
Home medical care may still seem like something special.
However, from the perspective of those working in the field:
it is becoming the most natural form of care for those who need it.

At such times,
whether or not one is aware of the option of home medical care
can greatly influence quality of life.

“a practice that supports a person’s daily life.”
And at the center of that care is always the patient’s life itself.
Home medical care is one way of staying close to that life.
