在宅医療という言葉は広く知られるようになりましたが、その仕組みや薬剤師の役割については、まだ十分に理解されていない部分もあります。
今回は、在宅医療の全体像と訪問薬剤師の具体的な働きについて整理してみたいと思います。

1章 医師の指示の下に動く在宅医療の仕組み
在宅医療は、各職種が独自判断で動く世界ではありません。
すべては医師の診療計画と指示の下で進みます。
訪問診療を行う医師が全身状態を把握し、治療方針を定めます。
その指示に基づいて、看護師が処置を行い、薬剤師が薬物治療を支えます。
私たち訪問薬剤師も、医師の処方に基づいてお薬を調剤し、ご自宅へお届けし、服薬指導を行っています。
そして服用状況、副作用の有無、体調変化を確認し、必要に応じて医師へ報告します。
「指示→実行→報告→再評価」という循環が、在宅医療の根幹です。
在宅医療は医療の質を下げるものではありません。
生活の場で医療を行うからこそ、より丁寧な観察と継続的な支援が可能になります。

2章 在宅訪問薬剤師の具体的な仕事
■ お薬手帳管理
在宅では複数医療機関を受診される方も多く、薬剤重複や相互作用のリスクが高まります。
お薬手帳は薬物治療の履歴書です。
処方の変遷を確認し、安全性を確保します。
■ 副作用モニタリング
薬には効果と同時に副作用の可能性があります。
・眠気
・ふらつき
・食欲低下
・便秘
・発疹
生活の中での変化を確認し、必要に応じて医師へ報告します。
■ 服薬アドヒアランス向上
薬が効くかどうかは、正しく飲めているかにかかっています。
飲み忘れや自己中断を防ぐため、生活リズムに合わせた服薬提案を行います。
■ 一包化
複数の薬を一回分ずつまとめる一包化は、高齢者医療では重要です。
飲み間違いを防ぎ、ご家族の負担を軽減します。
■ ポリファーマシー対策
薬の数が増えすぎると副作用リスクが高まります。
必要性を再評価し、減薬を提案することも薬剤師の役割です。

3章 メスを使わない治療の科学
薬物治療は、メスを使わない治療です。
外科手術とは異なり、化学的成分によって生体機能を調整します。
■ 薬理作用
薬は体内の受容体と結合し、特定の作用を引き起こします。
血圧を下げる、炎症を抑える、痛みを和らげるなど、精密な作用を持ちます。
■ 少量で効く理由
錠剤一粒に含まれる有効成分は数ミリグラム程度の場合もあります。

Chapter 1: The Structure of Home Medical Care Under a Physician’s Direction
Home medical care is not a system in which each profession acts independently based on its own judgment.
Everything proceeds under the physician’s medical plan and instructions.
The visiting physician evaluates the patient’s overall condition and determines the treatment policy. Based on that direction, nurses perform medical procedures, and pharmacists support pharmacotherapy.
As visiting pharmacists, we prepare medications according to the physician’s prescription, deliver them to the patient’s home, and provide medication guidance. We also assess adherence, check for side effects, monitor changes in physical condition, and report to the physician when necessary.
The cycle of “instruction → implementation → reporting → reassessment” forms the foundation of home medical care.
Home medical care does not reduce the quality of medicine. Rather, by bringing medical services into the patient’s living environment, it enables closer observation and continuous support.

Many home-care patients visit multiple medical institutions, increasing the risk of duplicate prescriptions and drug interactions.
The medication notebook functions as a résumé of pharmacotherapy.
We review prescription histories to ensure safety.

Medications provide therapeutic effects but may also cause side effects.

もうちょっと自分の中にある更新するハードルを下げたいと思いました。
ー医師の指導の下に動ける在宅医療
新聞で在宅訪問の看護師さんの働きを紹介する記事を見かけました。
看護師さんが自宅に来てくれるなんて、昔から考えたら夢のような世界です。
でも看護師さんたちも、なにも自主的にご自身たちが良いと思った看護行為を行なっているのではありません。あくまでも医師の指示の元に行動をして、医師に報告をしておられます。
ー薬剤師も医師の指示の下、動いてます
それと同様、私たち訪問薬剤師も、医師の指示を受けて、それに基づいてお薬をお届けして、服薬指導をさせていただいてます。
お薬手帳の管理、服用状況のチェック、痛みの様子、副作用症状の有無などです。
ー薬物治療は、言ってみればメスを使わない画期的な治療法です。
お薬を飲むということは、体にメスを入れて治療を行う外科的処置とは違い、お薬という化学的成分を持ったものを飲んで治療を行うということです。
一度飲んだからといって大きな治療効果がそれだけで出るわけではありませんが、継続して服用することで外科的な治療では出来ない療養をする事ができるわけです。
お薬は一粒で、数グラム、場合によっては数ミリグラムの量しか有効成分が入っていません。そんなほんのわずかな量のお薬が効果を発揮するのですから、お薬の持っている力はすごいと思います。
薬剤師の働きは、そんなお薬を正しく患者様にお届けし、適切に飲んでいただくためのお手伝いをさせていただく努めです。
今後もこの働きを通して1人でも多くの方々の健康的な生活をサポートしていきたいと願っています。

I recently came across a newspaper article introducing the work of visiting nurses.
Having a nurse come to your home—what a dreamlike idea, something unimaginable in the past.
But of course, nurses are not acting solely on their own judgment. They act strictly based on doctors’ instructions and report back to them.

Similarly, we visiting pharmacists also act upon doctors’ instructions. Based on those, we deliver medication and provide guidance on how to take it.
This includes managing medication notebooks, checking medication adherence, assessing pain levels, and checking for side effects.

Taking medication is fundamentally different from surgical treatment that requires incisions.
Instead, it is a form of therapy where healing is pursued by ingesting chemically composed substances.
Just taking one dose may not yield immediate dramatic results, but by continuing to take medication consistently, patients can experience healing that surgery cannot achieve.
Each pill typically contains only a few grams—or even milligrams—of active ingredients.
